Is your child interested in playing sports? Grades 5-8 may participate!
*Please return the sports physical form ANNUALLY completed by your private physician before the first day of your child's practice to obtain clearance in order to participate in school sports.
*All forms will be reviewed by the school physician, Dr. Frank and get clearance through this office.
These forms must be submitted in order for the student to participate in a school sport:

Please have your physician complete the following form and give it back to you to return to the school:
- Health History Update Questionnaire form (if > 90 days since last physical was completed). Parent completes this form. To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a "Health History Update Questionnaire" completed and signed by the student’s parent or guardian.
